The Practice Groups Board Committee has created a survey to assess the overall satisfaction among enrollees with the quality of benefits received as a member of the Practice Groups special "Electronic Access" offer.

As a member receiving electronic access to all Practice Groups in addition to those that you are fully enrolled in, you are receiving a copy of the survey.
