The Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Model’s first performance year began on January 1, 2023, and will end on December 31, 2026. The Centers for Medicare & Medicaid Services (CMS), through the Center for Medicare and Medicaid Innovation (CMMI), will initiate a new accountable care model that begins January 1, 2027, named the Long-term Enhanced ACO Design (LEAD) Model. The LEAD Model seeks to overcome the financial and administrative barriers that have caused health care providers to avoid or discontinue participation in accountable care organizations (ACOs).
