Academic medical centers (AMCs), safety net hospitals, and other hospitals across the country face significant challenges in discharging patients who require post-acute care. Rising patient acuity, limited behavioral health resources, and skilled nursing facility (SNF) capacity constraints frequently lead to "difficult to place" patients remaining in acute care beds longer than medically necessary.
